Report something unfriendly

Flag a book for the moderation team to review.

1. Open the book and click Report on its page.
2. Choose a reason and add any detail that helps a moderator understand.
3. Moderators see every report in a review queue.
4. If a book breaks the friendliness rules it can be unpublished.
